Sarcopenia does not wait for you to finish losing weight. It has been running quietly since your forties, taking somewhere around 1% of muscle mass a year, and a GLP-1 does not pause it.
Two subtractions, running at once
Losing 15 to 20% of body weight is a substantial intervention at any age. After 50 it lands on top of a process already removing muscle each year.
The combination is why the NEJM work on weight loss in older adults kept finding the same thing: weight loss alone improved some measures and worsened others, and it was weight loss with exercise that improved physical function without costing lean tissue.
Ranked · GLP-1 supplements for adults over 50
How this was ordered: Weighted for two processes running at once — age-related sarcopenia and drug-driven weight loss. Anything protecting muscle and bone ranks above everything else. Protein requirements rise with age, and absorption of B12 falls, so both are treated as needs rather than optional extras.

1. GLP-1 Foundation
Creatine's evidence is arguably strongest in exactly this group — older adults doing resistance training show consistent gains in lean mass and functional strength, and this is the population with the most to lose. Losing 20% of body weight after 50 without a deliberate muscle strategy is how people arrive at frailty by a route they did not expect.
Against it The 1,000 mg sodium load needs a conversation if you are treated for blood pressure or heart failure, which is far more common in this age group. No independent certification, and anyone with reduced kidney function should clear creatine first.
2. Gold Standard 100% Whey
Older adults need more protein per kilogram than younger ones to trigger the same muscle protein synthesis — anabolic resistance is real and well documented. Combine that with GLP-1 appetite suppression and liquid protein stops being optional.
Against it Higher protein loads warrant care in anyone with reduced kidney function. Worth a conversation and a recent eGFR rather than an assumption.
3. Vitamin D3
Rapid weight loss reduces bone density and older adults are already losing it. Vitamin D at a few cents a day supports bone and, in deficient older adults, muscle function. Nothing on this page offers a better return per dollar.
Against it Only meaningfully helps if you are actually low. Test rather than assume, and pair it with adequate calcium.
4. Basic Nutrients 2/Day
B12 absorption declines with age and declines further with long-term metformin or acid suppression, both common in this group. A well-formulated multivitamin covers that alongside the general gaps of a much smaller plate.
Against it A pharmacy multivitamin costs a fifth as much and covers most of the same ground. You are paying for form quality and certification.
5. GLP-1 Companion
Constipation is more common with age before any drug is involved, and a named-strain synbiotic is a reasonable daily maintenance addition.
Against it Fibre and fluid remain cheaper and more effective. Check your existing medications first — constipation in this group is often iatrogenic rather than dietary.

Anabolic resistance, briefly
Older muscle responds less readily to the same amount of protein. It takes more leucine, and therefore more total protein, to trigger the same synthetic response.
Which produces an unfortunate collision: your requirement goes up with age, and your appetite goes down with the drug. That is the case for liquid protein in this group, and it is a stronger case than for anyone else.
The test that changes the answer
Before adding anything here, two numbers are worth having: kidney function and vitamin D status.
The first determines whether high protein and creatine are appropriate for you. The second determines whether the cheapest item on this page is doing anything at all. Both are routine, both are inexpensive, and both convert guesswork into a decision.
